Regular or Primary Conference Delegate:
Registration for Primary Delegates includes all breakfasts, lunches, and museum/library visits from Monday to Wednesday, and dinner on Wednesday evening. If you are the Primary Delegate attending the conference, please use this form to complete your registration. If you are bringing a guest, please complete the Primary Delegate form first, then complete a separate Guest form (below) for each guest as described below. All registrations (Primary, Guest, Sponsor) must be completed and paid individually.
Guests:
If you are registering a Personal Guest, please complete the Primary Delegate form above first, then use this form to register your Guest. Please complete this form only after you have completed and submitted payment for yourself as the Primary Delegate. All registrations (Primary, Guest, Sponsor) must be completed and paid individually.

If you are a Sponsor, you will receive sponsor registration information directly from the Local Planning Committee.
